These Terms of Use govern your use of adhdforall.com (the "Site"), operated by [Practice legal name], doing business as ADHD for All. By using the Site, you agree to these terms.
Content on the Site is for general information. Browsing the Site, taking the free check, or submitting a registration form does not create a clinician–patient relationship; that relationship begins only when you complete registration and are seen by one of our licensed clinicians. The two-minute check is an educational screening based on commonly used questions. It cannot diagnose ADHD or any condition, and its result does not guarantee that you qualify for, or will receive, any particular diagnosis, treatment, medication, or documentation. Only a licensed clinician can diagnose ADHD, after a full evaluation.
Clinical services are available only where our clinicians are licensed, are subject to clinical judgment, and are governed by the consent forms you sign during registration and intake. Prescriptions, accommodation letters, and other documentation are issued solely at the clinician's discretion based on evaluation findings.
You agree to provide accurate, current information when registering. If the patient is under 18, registration must be completed by a parent or legal guardian, who is responsible for the minor's use of our services.
The first visit is a flat $100 and follow-up visits are $75, payable per visit; there is no membership. Medication costs are separate and depend on your pharmacy and coverage. Payment links are sent to your registered email — if you are ever unsure a payment request is genuinely from us, verify with our care team before paying.
You agree not to misuse the Site — including attempting to breach security, scrape content, impersonate others, or submit false information.
Scheduling (Healow), video visits (Doxy.me), and certain form-delivery functions are provided by third parties under their own terms and privacy policies, which we do not control.
The Site and its content are owned by us or our licensors and may not be copied or reused without permission, except for your personal, non-commercial use.
The Site is provided "as is" without warranties of any kind. To the fullest extent permitted by law, we are not liable for indirect, incidental, or consequential damages arising from use of the Site. Nothing in these terms limits liability that cannot be limited under applicable law, and nothing here alters our professional obligations to patients under medical law.
